When You Have Both Medicare and NJ FamilyCare
Some Irvington residents qualify for Medicare and for Medicaid, which New Jersey calls NJ FamilyCare. People in this situation are called dual eligible, and they often juggle two cards, two sets of rules and two customer service lines.
Dual Eligible Special Needs Plans, called D-SNPs for short, are Part C plans built around this exact situation. In New Jersey, these plans are fully integrated, meaning a single company manages both your Medicare and your NJ FamilyCare benefits.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
How a D-SNP Helps
- One member card and one plan for medical, hospital, drug and Medicaid services
- Very low or no copays for many covered services
- A care coordinator who helps arrange appointments and services
- Add-ons like dental, eyeglasses, hearing aids or a pharmacy-aisle allowance, which vary by carrier
- Coordination of home care and other long-term support when you are eligible
For a person with several conditions, a care team that sees the whole picture can make a real difference.
Who Qualifies
Eligibility usually requires enrollment in both A and B, full NJ FamilyCare, and an address inside a carrier's coverage territory; several carriers include Essex County. People who receive only partial state help, for instance through a Medicare Savings Program, may see other choices.
Not sure about your status? Your most recent NJ FamilyCare letter usually shows it. We can help you read it, and our savings programs page explains the partial-benefit categories.
Changing Plans as a Dual Eligible
Dual eligibles enjoy extra flexibility. Federal changes that took effect in 2025 created a once-a-month opportunity to join an integrated plan such as a D-SNP, alongside the usual fall Open Enrollment.
That flexibility is helpful, but it also means more calls from people trying to switch you. Check every offer against your doctors and medicines before agreeing, and never share your Medicare or Medicaid number with an unknown caller.
Checking Doctors and Services
D-SNPs use networks. Before joining, we confirm your primary care doctor, specialists, hospital preferences such as University Hospital or Newark Beth Israel, and any home care agency or day program you use.
If you rely on personal care assistance or medical day services, we check how each plan handles them so care is not interrupted.
Talk It Over at No Charge
Have your medication list and your doctors' names handy. We set your current coverage beside the options we carry and show what changes in dollars and in access to care. Most calls finish in about half an hour, nobody asks you to commit on the spot, and you can learn about our agency beforehand.

Does joining a D-SNP end my NJ FamilyCare?
It does not. Your state benefits remain, and the plan simply runs them in step with Medicare.
Can my caregiver help with enrollment questions?
Yes. With your permission, a family member, home aide or caseworker can join the call.
Is there a cost to join?
Many dual eligible members pay no premium, but plan details vary, so we check each one.
What if I lose NJ FamilyCare?
D-SNPs usually allow a grace period while you reapply. Contact the plan and the county Board of Social Services right away.
